If you are scheduled for your routine annual eye exam, please complete the paperwork below:

Returning Patient

A patient is considered a returning patient if they have been seen within 3 years (2 forms to complete, 1 optional)

If you wear contact lenses, please also complete the following (1 additional form):

New Patient

New Patient OR if it has been 3 years or more since we have last seen you (2 forms to complete, 1 optional):

If you wear contact lenses, please also complete the following (1 additional form):

A contact lens evaluation is necessary if you would like a new or renewed contact lens prescription, even if you have done one in the past. If you decline a contact lens evaluation and your prescription is expired, you will be unable to order more contact lenses.
